About the Nurse Managed Clinic

The purpose of the Nurse Managed Clinic is to provide primary healthcare to the medically under served and income eligible residents of Switzerland County and surrounding areas who are unable to access such care due to financial status or a lack of medical insurance.

The clinic shall also provide care to the insured individuals and those enrolled in Medicare and Medicaid wishing to access clinic services.

What if I don't bring the income information?

You will be charged the full charge for the visit until you can provide this information.

What if I don't have insurance?

Then the State requires that we get some sort of proof of income (check stub, tax papers, disability information). We use this to decide if you meet the sliding-fee scale for the office charges.

What information do I need to bring to my first visit?

If you have insurance, you will need to bring your card. Also a list of medicines, including doses, would make the visit go much smoother.

What is a Nurse Practitioner?

A nurse practitioner (NP) is a registered nurse (RN) who has advanced education (a minimum of a master's degree) and training in the diagnosis and management of common medical conditions, including chronic illnesses. Nurse practitioners provide a broad range of health care services. They provide some of the same care provided by physicians and maintain close working relationships with physicians. An NP can serve as a patient's regular health care provider.

Nurse practitioners see patients of all ages. Nurse practitioners focus on patients' conditions as well as the effects of illness on the lives of the patients and their families. NPs make prevention, wellness, and patient education priorities. This can mean fewer prescriptions and less expensive treatments.
