FREE MAMMOGRAMS

The Switzerland Co Health Dept is providing free mammograms to women age 40 and over or under age 40 who have a history of breast cancer in the immediate family. This program is provided by a grant from Indiana Breast Cancer Awareness. IBCAT receives funds through sales of breast cancer awareness specialty license plates. Through these sales, monies are available for grants deemed to best address the unmet screening needs of the people of Indiana.
If it has been at least a year since your last screening call the Switzerland Co Health Dept and ask for Betty Lucas or Donna Faber at 812-427-3220 for more information on scheduling your appointment. This program will run from March 17 -October 31, 2014 or until appointments are filled.

NEW IMMUNIZATION REQUIREMENTS

New requirements for immunizations for the 2014-15 school year.
3 TO 5 YEARS OLD:
3 Hep B, 4 DTaP, 3 Polio (Inactivated Polio), 1 MMR, 1 Varicella
KINDERGARTEN:
3 Hep B, 2 MMR, 5 DTap, 2 Varicella, 4 Polio*, 2 Hep A
GRADES 1 TO 5:
3 Hep B, 2 MMR, 5 DTaP, 2 Varicella, 4 Polio
GRADES 6 TO 11:
3 Hep B, 2 MMR, 5 DTaP, 2 Varicella, 4 Polio, 1 Tdap, 1 MCV4
GRADE 12:
3 Hep B, 2 MMR, 5 DTaP, 2 Varicella, 4 Polio, 1 Tdap, 2 MCV4
Hep B Two dose alternative adolescent schedule is acceptable if properly documented.
DTaP Four doses of DTaP/DTP/DT are acceptable if 4th dose was administered on or after child's fourth birthday.
Polio Three doses of polio vaccine are acceptable if 3rd dose was administered on or after child's fourth birthday and the doses are all IPV or all OPV.
*The 4th dose of polio vaccine MUST be administered on or after child's fourth birthday.
MMR If given as single antigen, 2 Measles, 2 Mumps and 1 Rubella required.
Varicella Physician documentation of disease history, including month and year, is proof of immunity.

Do I need to call for an appointment for immunizations?
At this time we do not make appointments for immunizations. It is strictly walk-in only. The nurse is available for these from 8:00-11:00 am and 12:00-3:30 pm on Mondays, Tuesdays, Wednesdays and Fridays and 8:00am to 12:00pm on Thursdays.
Birth and Death Certificates
Birth and Death Certificates have to be applied for in the county in which the birth or death occurred or through Indiana Vital Records.
Switzerland Co charges $10 for each certified copy.
