NEW IMMUNIZATION REQUIREMENTS

The Switzerland Co Health Dept wants to remind everyone of the new requirements for immunizations for the 2013-14 school year.
*3-5 yr olds: 3 doses Hepatitis B, 4 Dtap, 3 Polio, 1 MMR, 1 Chicken Pox (or proof of disease history)
*Kindergarten: 3 doses Hep B, 5 Dtap, 4 Polio, 2 MMR, 2 Chicken Pox (or proof of disease history), 2 Hep A-this is recommended but not required
*Grades 1-5: 3 doses Hep B, 5 Dtap, 4 Polio, 2 MMR, 2 Chicken Pox (or proof of disease history)
*Grades 6-10: 3 doses Hep B, 5 Dtap, 4 Polio, 2 MMR, 2 Chicken Pox (or proof of disease history), 1 Tdap, 1 Meningitis
*Grades 11-12: 3 doses Hep B, 5 Dtap, 4 Polio, 2 MMR, 2 Chicken Pox (or proof of disease history), 1 Tdap, 2 Meningitis-second is recommended but not required.

You may get your vaccinations at the Health Dept if you meet eligibility requirements of one of the following:
1. You have no health insurance.
2. You have private insurance, but insurance DOES NOT pay for vaccines.
3. You have Hoosier Healthwise or Medicaid.
Otherwise, you will need to schedule an appointment with your healthcare provider.
Please bring a copy of your child's shot record and Medicaid card with you when coming to the Health Dept.

Hours we offer vaccinations are: Monday, Tuesday, Wednesday and Friday
8:00 am - 10:30 am and 12:00 noon - 3:30 pm or Thursdays 8:00 am - 11:00 am.

Do I need to call for an appointment for immunizations?
At this time we do not make appointments for immunizations. It is strictly walk-in only. The nurse is available for these from 8:00-11:00 am and 12:00-3:30 pm on Mondays, Tuesdays, Wednesdays and Fridays and 8:00am to 12:00pm on Thursdays.
Birth and Death Certificates
Birth and Death Certificates have to be applied for in the county in which the birth or death occurred or through Indiana Vital Records.
Switzerland Co charges $10 for each certified copy.
